Please click on the link below for information on the Florida Immunization Requirement 2011-2012. 

In addition to immunization requirements, physical examinations are required for Palm Beach County students entering grade seven and must be presented to the office of the school the student will be attending on the DH Form 3040.  Physicals for all 7th grade students must be dated August 22, 2010 or later. Please submit any updates to your child’s health records to the school the child will be attending as soon as possible.  You can present records any time prior to the first day of school on August 22, 2011.

Remember, even virtual schools must follow the “No Shots, No School, No Kidding” health guidelines.  Students will not be able to begin Fall classes until the requirements are met.
